17 years/Male having a history of complex partial seizures, at 10-15 days intervals despite treatment for 12 years. His routine activities including schooling and games were significantly compromised d/t frequent seizures.
MRI brain was s/o subtle FLAIR hyperintensity in Lt. Hippocampus with likely possibility of mesial temporal sclerosis, seizure semiology & video EEG were s/o Lt. Hemispheric focus. PET-CT revealed diffuse asymmetrical severe hypometabolism involving Lt. temporal lobe. In view of clinical, radiological, electrophysiological, & PET CT correlation surgery was planned.
After dural opening brain mapping was done with surface electrodes, and ECoG revealed an epileptogenic area over the entire middle and inferior temporal gyrus. Lt Temporal lobectomy with Amygdalo-hippocampectomy was done. At the completion, a completely normal ECoG was achieved.
Post-op remains uneventful & patient was discharged on the 7th post-op day.
